Instrumentation Laboratory GEM Premier 5000 PAK recall: Confirmed customer complaints indicating that GEM PAKs (cartridges)

Recall published 7 months ago

Confirmed customer complaints indicating that GEM PAKs (cartridges) for the GEM Premier 5000 may experience an increased incidence of Process Control Solution Not Detected (PCSND) errors during warm-up, including consecutive occurrences, resulting in GEM PAK ejection and requiring insertion of a new GEM PAK. iQM2 is an active quality process control program designed to provide continuous monitoring of the entire testing process. Thus, if a GEM PAK completes AutoPAK Validation following warm-up, it may remain in use. Consecutive GEM PAK ejections during warm-up may prolong turnaround times, potentially delaying results. In such cases, patient management may require reassessment once results are available. While many GEM PAKs continue to perform as intended, consecutive ejections may increase the likelihood of operational disruption, highlighting the importance of advance planning where feasible to help minimize impact.

The hazard: Impact or projectile hazard

This product can throw a part or strike the user, which is why the recall is phrased as an impact hazard rather than a breakage complaint.

Why it matters: The product can throw a part, release stored energy unexpectedly or strike the user. Eye injuries are the common outcome, and they are frequently permanent.

Symptoms and when to call a doctor

For an eye injury, do not rub the eye and do not try to remove anything embedded in it. Cover the eye with a shield that does not press on it — the bottom of a paper cup taped in place works — and get to an emergency department. For a blow to the head, watch for worsening headache, repeated vomiting, confusion or drowsiness, and call 911 if any of those appear.

What to do about Impact

Instrumentation Laboratory — This product can throw a part or strike the user, which is why the recall is phrased as an impact hazard rather than a breakage complaint.

  1. Stop using the product and release any stored tension or pressure if the instructions describe a safe way to do it.
  2. Follow the remedy on this page and quote the model and date code from the label.
  3. Keep bystanders, and particularly children, away from it until the remedy is complete.
  4. Do not substitute your own parts or fasteners as a workaround.

Claiming your refund, repair or replacement

The recalling company decides the remedy and funds it. You should never pay for a recall repair or for return shipping.

  • Use the company contact on this page first. Most firms run a dedicated recall page with a claim form that is far quicker than their main support line.
  • If you bought it from a retailer rather than the brand, try both. Target, Walmart and Amazon often refund recalled goods from the order history.
  • If the company stops responding, escalate to the CPSC. Firms under a recall order have reporting duties to the agency.
  • Have the model number, the date code and your photos ready. Many companies issue store credit by default — ask for cash or a card reversal if you would rather have it.
